Ethics approval and consent to participate
: The Hunter New England Health Human Research Ethics Committee approved the study, reference no. 11/02/16.5.01; HREC/10/HNE/402; SSA/10/HNE/402 in February 2011. The ethics committee did not require consent from individual patients as data were routinely collected as part of patient care through administrative hospital data. Individual patients were not approached and data was de-identified.
