: This study was approved by the Research Ethics Board at Sunnybrook Health Sciences Centre and the University of Waterloo, Office of Research Ethics (ORE File #19950). Informed consent by participants was not required. ICES is a prescribed entity under section 45 of Ontario’s Personal Health Information Protection Act.
