: Ethical approval for the study was granted by the internal review board at the Medical Faculty of the University of Freiburg, Germany (no. 495–17, date: 19th December 2017). Written informed consent will be requested from subjects or their legal representatives before enrolment. If a patient has a legal guardian, proxy consent is used.
