: The study protocol was approved by the Ethics Committee of the Turku University Hospital and it complies with the ethical rules for human experimentation that are stated in the Declaration of Helsinki. The patient or his/her trustee was informed about the study both orally and in a written form. Patient’s previously named trustee was approached, if patient’s ability to independent decision making was deteriorated.
