: All procedures performed in studies involving human participants were in accordance with the ethical standards of the institutional committee. The ESA-S project was reviewed and approved by the research ethics board of the CIUSSS de l’Estrie–CHUS (Projet #2012–03). Access to the health-administrative database was approved by the Commission d’accès à l’information du Québec (CAIQ) (#110368).
