Ethics approval and consent to participate
: Written consent was obtained for participation in the CFAS study. Ethical approval was obtained locally at all sites from 1991 and at Multi-centre research ethics committees during the course of the study (further detail on all ethical approvals can be found at ExternalRef removed).
