Ethics approval and consent to participate
: This study has been assessed by the Institutional Review Board of the University Medical Center Groningen (UCMG), which stated that no approval was needed as this non-invasive study was not subject to the Dutch Medical Research Involving Human Subjects Act (METC decision: METc 2014/405). All participants of the focus groups have consented to the participation in and audiotaping of the interviews. Verbal informed consent was recorded on tape in line with the then valid rules and regulations for research as used by the UMCG. The interviews were transcribed and analyzed with anonymized codes.
