Ethics approval and consent to participate
: According to the Dutch Medical Research Committee Zuyderland, ethical approval was not required (METC #17 N110). All focus group participants provided written informed consent. For the remaining participants, no written informed consent was needed since the provision of the program was considered as a regular quality improvement strategy. Nevertheless, all participants received detailed information about the aim of the study in advance, and participation was voluntary; participants could withdraw from the study at any moment without providing a reason.
