: At the Rijnstate hospital, all patients have been informed that routine registration data are anonymized and can be used for scientific research; if they don’t want their data to be used, they can opt-out. This study was performed following Good Clinical Practice guidelines and the Dutch law (<i>Wet op de Geneeskundige Behandelingsovereenkomst WGBO</i> [CitationRef removed] and <i>Wet Maatschappelijke Ondersteuning WMO</i> [CitationRef removed]).
