Ethics approval and consent to participate

: Ethical approval for this qualitative study was sought as a substantial amendment to the main PreFIT study (REC reference: 10/H0401/36). The amendment was approved by Derby Research Ethics Committee in August 2017. All participants provided written, informed consent. They were informed before data collection and reminded at the start of each interview that their participation was voluntary, and they were free to withdraw at any time without prejudice. All participants’ names have been replaced with pseudonyms to protect confidentiality.
