: The study was conducted in accordance with the Tri-Council Policy Statement, Ethical Conduct for Research Involving Humans []. Institutional ethics approval was obtained from: the McMaster University Hamilton Integrated Research Ethics Board (REB) (# 8017) and the Hotel Dieu Shaver Health and Rehabilitation Centre REB and renewed yearly, as required.
