: The study was assessed by the Medical Ethics Committee of University Hospital Maastricht/Maastricht University (reference number 2023–3744), and they confirmed that the Medical Research Involving Human Subjects ACT (WMO) does not apply to this study and, therefore, an official approval of this study by the Medical Ethics Committee is not required. Written informed consent was obtained from the participants in the Better@Home cohort (patients, informal caregivers, and the care professionals who participated in the questionnaire and/or interview). All data of the patients in the Care as usual cohort is received from the GR facilities in an anonymized way, so it cannot be linked to specific persons by the research team.All procedures in this study involving human participants are in accordance with the recent version of the Declaration of Helsinki. All participants are covered by insurance while participating in the study. Written informant consent for study participation is obtained from all participants during inpatient rehabilitation before study inclusion and baseline assessment.
