: We conducted the study in accordance with the World Medical Association Declaration of Helsinki and the Regional Committee for Medical and Health Research Ethics of Western Norway approved the study prior to its start, in line with the Norwegian legislation- the research ethics act and the Health Research act. This committee is appointed by the Norwegian Ministry of Education and Health. The patients received written information with a possibility for reservation from participation in the study after discharge, as approved by the Regional Committee for Medical and Health Research Ethics (Western Norway; REK 424462). Patients who fulfilled inclusion criteria and died during or shortly after the study period were directly included as approved by the Regional Ethics Committee. The patients did not need to consent to participate in the study, this was approved by the Regional Committee for Medical and Health Research Ethics.
