: Most data used in the model came from publically available and/or published data; use of this data is exempt from ethics review in the U.S. . However some data came from the 4 Pillars cluster randomized trial. In that study, standard informed consent procedures were followed, and trial protocols were approved by institutional review boards of the University of Pittsburgh, Baylor College of Medicine, and the Harris Health System.
