: The permission to conduct the study was obtained from each universityin Sri Lanka and the study protocol was approved by the Institutional Ethical Review Committee, Faculty of Medicine, University of Peradeniya, Sri Lanka (Reference number 2015/EC/82) prior to the data collection. Written informed consent was obtained from every participant and no financial inducements were offered. The participants were assured of the confidentiality and anonymity of the information they provide.
