Ethics approval and consent to participate

: This study was granted exemption from full ethics approval by Central Coast Local Health District (CCLHD) human research ethics committee as this study met all criteria as a Quality Improvement audit. The approval for this study as a Quality Improvement audit was given by the Director of Clinical Governance CCLHD (reference - EC 0118-003C). The need for participant consent was waived by the CCLHD HREC as this study qualified as a QI audit.
