Ethics approval and consent to participate

: Ethical clearance for the study was sought and obtained from the institutional review board of the Faculty of Medicine and Pharmaceutical Sciences (Reference number IEC-UD/488/02/2016/T). All aspects of the methodology were clearly defined in the process. Verbal informed consent was obtained from patients or their next of kin before they were interviewed over the phone. Administrative clearance from the DGH permitting use of patient files was also obtained. Confidentiality was maintained throughout the study.
