: This study was approved by the ethics committee of Ikeda Memorial Hospital (ethical approval number is currently not available because no specific approval number is provided to each case report at Ikeda Memorial Hospital). Written informed consents were obtained from the patient and all his family members including his spouse, parents, and the spouse’s parents. The patient gave written consents on behalf of his son and daughter whose ages were < 18 years.This study was conducted in accordance with the tenets of the Declaration of Helsinki.
