: A favourable ethical opinion to perform the whole of the study was granted by the North West Research Ethics Committee (07/MRE08/5). Research management and governance was then sought, and approval granted by all relevant Research and Development Organisations across the United Kingdom [ref: O’Brien et al. BMC Medical Research Methodology 2010, 10:39].The IID2 study obtained and recorded oral informed consent from participants in the Telephone Survey using the CopyCall Telephone Recorder. We obtained written informed consent from all adults in the prospective studies. We obtained written informed assent from children and written informed consent from their parent or guardian.
