: Approval for the study was provided by the Review and Ethics Committee (REC) of the Federal Teaching Hospital, Abakaliki (REC protocol number 07/10/2016–07/10/2016; REC approval number 07/10/2016–07/11/2016; FETHA/REC/VOL1/2016/435). An informed consent form was signed by parents/guardians of the children prior to enrolment in the study; information in the form made clear that participation in the study was voluntary and participants could opt out of the study at any time without prejudice to the quality of treatment received by their children. Questionnaire data were coded to be anonymous such that it is not possible to identify individual patients.
