Ethics approval and consent to participate
: All study protocols and informed consent forms were approved by the Biomedical Research Ethics Committee at the University of KwaZulu-Natal as well as the various study-specific Institutional Review Boards. Ethical approval for the trials were received from the University of KwaZulu-Natal Biomedical Research Ethics Committee and the South African Medical Research Council.
