: Ethical approval for retrospective and prospective analysis of these data was obtained from Bugando Medical Centre in Mwanza (BREC/001/04/2011), the National Institute for Medical Research in Dar es Salaam (NIMR/HQ/R.8a/Vol.IX/2446), and Weill Cornell Medicine in New York (1108011883).
