Ethics approval and consent to participate
: Ethical approval for the RCT and qualitative sub-study were provided by UCL Research Ethics Committee (ref: 11945) and LSHTM Observational Research Ethics Committee (refs 9233/001). SELPHI was prospectively registered with the ISRCTN on 10/08/2016 (ref: ISRCTN20312003). All participants provided informed consent ahead of their involvement with the RCT and qualitative sub-study. Due to the remote nature of these studies, RCT consent was online written consent; sub-study consent was verbally recorded at the beginning of each interview.
