: All participants signed/will sign informed consent prior to participation (see Additional file for full form). Participation will require no additional expenses and therefore participants will receive no compensation for partaking in the study.Ethics approval has been granted by the University of Cape Town (HREC 652/2016) and the MSF Ethics Review Board (protocol #1639). The research committee of the Provincial Government of the Western Cape Department of Health also approved this study (WC_2016RP41_532). The study was registered with the Pan African Clinical Trial Registry (PACTR201810631281009) on 11 September 2018.
