Ethics approval and consent to participate
: The study received ethical approval from the THIN Scientific Review Committee (16THIN103). THIN data was received by Public Health England in a pseudo-anonymised format and stored on secure servers. JT was given access permissions to the raw data; this permission was granted by the above approval and by internal PHE data access agreements. No one in the team was granted administrative permissions.
