Ethics approval and consent to participate
: The EndoAfrica study has received ethics approval from the Health Research Ethics Committee (HREC) of Stellenbosch University (reference numbers: N13/05/064 S16/07/114), and the EndoAfrica-NWU study of HREC of the North-West University (reference number NWU-00045-15-A1). The study was conducted according to the ethical guidelines and principles of the international Declaration of Helsinki, South African Guidelines for Good Clinical Practice and the Medical Research Council of South Africa Ethical Guidelines for Research. All the participants signed the informed consent form after a detailed description of the study and planned measurements were conveyed to them in their native language. Participants were allowed to withdraw from the study at any time.
