Ethics approval and consent to participate
: The study was completed in accordance with the Declaration of Helsinki and approved by the Human Research Ethics Committee, Faculty of Health Sciences, University of Cape Town, reference number: HREC 107/2017. Furthermore, the hospital research committee approved the study. Informed consent was not obtained from individual patients or caregivers because the data was collected retrospectively. Patient details were anonymised before data analysis.
