: The study protocol was approved by the Medical Ethics Committee of the University of Taiz, Yemen. Verbal consent as permitted by the Ethical Committee was obtained from each participant after adequate explanation of the nature and objectives of the study. Verbal consent was obtained instead of written consent because of concerns about protecting participant confidentiality considering the tribal tensions and unrest in the study area.
