: As this is a retrospective study, patient care or management was not be affected. Individual patients consent was waived by the superintendents of each healthcare facility. The study was approved by the University of Cape Town's Research Ethics Committee and research clearance was granted by the two facility research committees (GSH HREC 485 and RCH’s RCC 153). All methods were performed in accordance with the relevant ethics guidelines and regulations.
