: Ethics approval was obtained from the Research Ethics Committee of the Federal University of São Paulo (UNIFESP-CEP). All informants provided written informed consent. We did not have illiterate participants in our study because all participants were able to read and sign the informed consent documents (information sheet and consent form). Where the tables indicate ‘Education: None’ we aim to report that ‘no formal education was completed’ but they were literate. We did not have special measures for illiterate participants (i.e., legal guardian, witnesses etc.) because no illiterate persons participated in our survey. All methods were carried out in accordance with relevant guidelines and regulations.
