: The study was performed in accordance with the Declaration of Helsinki and was approved by the School of Biomedical Sciences-Research and Ethics Committee, College of Health Sciences, Makerere University (approval number: SBS-HDREC-650), Mulago National Referral Hospital-Research and Ethics Committee (approval number: MHREC-1702), and Uganda National Council for Science and Technology (approval number: HS411ES). It was also approved by the Joint CUHAS/BMC Research and Ethics Committee (CREC/409/2019) and the National Health Research Ethics Review Committee of the National Institute for Medical Research (NIMR/HQ/R.8a/Vol.IX/3322) in Tanzania. All information obtained was coded and kept confidential. Written informed consent was obtained from all the participants and from the legal guardians of minor and illiterate participants. For participants aged between 12 and 17 years, their assent was also obtained. Illiterate participants added their fingerprints to the consent form along with an impartial witness who had also signed and was present during the informed consent process.
