: This study was approved by the national research ethics authorities through the Regional Committee for Medical and Health Research Ethics Western Norway (REK West) with the reference number 135576. All procedures involving human participants were conducted in accordance with the ethical standards of the responsible institutional and/or National Research Committee and with the tenets of the 1964 Helsinki Declaration and its later amendments and comparable ethical standards. The informed consent was waived by the Regional Committee for Medical and Health Research Ethics Western Norway (REK West), ref. nr 135576, because of the nature of the study using data reported to national registers and that the data were handled so the welfare and integrity of the participant were safeguarded. The data were de-identified by register owners using a project-specific ID for this study and made available for this study without possibility to identify any individuals.
