: This study was approved by the Ministry of Health of Kenya (MOH), Scientific and Ethical Review Unit (SERU) of the Kenya Medical Research Institute (KEMRI, KEMRI/CGMR-C/203/4085), the Oxford Tropical Research Ethics Committee (OxTREC 44 − 20), and the London School of Hygiene and Tropical Medicine Research Ethics Committee (LEO 26950). Additional approval was sought from all participating hospitals and their respective county health departments.
