This study was admitted and approved by the Institutional Review Board (IRB), i.e., Lund Regional Ethics Committee (Nos. 2016/220). Informed consent was waived, and patients were offered an option out by the IRB i.e., Lund Regional Ethics Committee (Nos. 2016/220) due to the retrospective nature of the study.
