Before the data collection, ethical approval was obtained from the Institutional Review Board (IRB) of the College of Health Sciences, Mekelle University (IRB reference number: MU-IRB 2172/2024). An official permission letter was obtained from the clinical director’s office and distributed to the quality assurance and HIMS departments. For the sake of confidentiality, no specific identifiers such as the name of the patient, images, or voice records were used. Furthermore, according to the IRB of the College of Health Sciences, Mekelle University, consent to participate is not required (not applicable) for retrospective studies that use secondary data from the electronic and document records of health facilities.
