: Ethical approval for the conduct of this study was obtained from the Navrongo Health Research Centre Institutional Review Board (Approval ID: NHRCIRB532). Written permission was obtained from the regional director of health service of the Upper East Region and the managers of the selected health facilities. Informed consent was obtained from participants or their parents/guardians for patients aged between 2 and 17 years prior to the commencement of study activities. In addition to the consent from their parents/guardians, assent was obtained for children aged between 12 and 17 years. All study procedures were conducted in conformity with the Declaration of Helsinki.
