: Ethical approval for the study was obtained from the ethical committee of the Wallaga University Institute of health Science, Institutional Health Research Ethics Review Committee (IHRERC). Letter of the permission was obtained from the Wallaga University Public health department to get permission from Nekemte town health Office. Then, a supportive letter was obtained from the town health office to get permission from the administrative office of the public health care facilities. Informed voluntary, written, and signed consent was obtained from each head of the healthcare facility. To assure confidentiality, the name of the patients was not recorded in the data extraction check list rather the identifiers of patients participated in the study was coded and data was kept secured. The recorded data was not accessed by a third person, except the principal investigator.
