Ethics approval and consent to participate

: The study was approved by the institutional review board (IRB) at the American University of Beirut (protocol number: Med.IB.01). All subjects gave written informed consent in accordance with the Declaration of Helsinki. Children and adolescents under age of 16 signed an assent form, and got their parents’s written consents to be included in the study. Genetic analyses and return of genetic data were performed in accordance with protocols approved by the Partners Human Research Committee.
