: The ACTIVE Dialysis study was carried out in accordance with the principles of the Helsinki Declaration and approval was obtained from Human Research Ethics Committee of Northern Sydney Central Coast Health, NSW, Australia (HREC/09/HARBR/26). Approval was also obtained from each participating site according to their local practice. Outside Australia, the study was approved by Peking University Institutional Review Board, China (IRB00001052–11079); Multi-region Ethics Committee, New Zealand (MEC/09/07/078) and University Health Network Research Ethics Board, Canada (09–0820-BE).
