: This manuscript contains survey data of VHA dialysis staff (medical directors and nurse managers) to assess their use, satisfaction, and application of the Dialysis Dashboard to Veteran care. The VHA National Kidney Disease and Dialysis Program office conducted this confidential electronic survey (questionnaire). Participation and completion of this survey by dialysis staff were completely voluntary and participants were assured that all individual survey results would be kept confidential and no identifying information would be used in reports. The Edward Hines VA Institutional Research Board (IRB) formally reviewed this project protocol and all accompanying products from it. Edward Hines VA IRB determined that it does not meet criteria for research under the Common Rule at 38 CFR 16.102 and HHS at 45 CFR 46, and therefore is not required to have formal IRB review and approval for research. Additionally, informed consent was deemed unnecessary by the Edward Hines VA IRB because this employee survey was completely voluntary and not considered research.
