Ethics approval and consent to participate
: Each phase of research (patient focus groups, provider interviews, and patient cognitive interviews) was reviewed and received ethics approval from Ethical and Independent Review Services, Inc. (). The review/approval numbers for the research are: patient focus groups (#17120–01), HCP interviews (#17177–01) and patient cognitive interviews (#18049-01A). Written consents were obtained for all participants in the focus groups. IRB approval was granted for verbal consents for the provider interviews and patient cognitive interviews since these were not conducted in-person.
