KBKA /34/O/2023), maintaining due diligence in protecting personal data in accordance with applicable Polish law, has approved the study on June 15th 2023. I agree to the processing of my personal data included in the questionnaire by Andrzej Frycz Modrzewski Krakow University for the purpose of participating in the study entitled “Attitudes and Knowledge of Dialysis Patients Regarding Kidney Transplantation.” In accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data ("General Regulation") and the Polish Personal Data Protection Act of May 10, 2018 (Journal of Laws 2018, item 1000), I acknowledge that this consent may be withdrawn at any time by submitting a statement of intent to Andrzej Frycz Modrzewski Krakow University.
