: All the participants provided informed written consent prior to their enrollment in the study, which included clear information about potential of publication of anonymized data. Participants were informed that no identifiable personal information will be disclosed in any publications of the study. Given that many participants reside in distant and rural areas and may not return for follow-up, it was not feasible to obtain re-consent specifically for manuscript publication. : Patients were not involved in the initial design, recruitment or conduct of this study. The research questions and study objectives were informed by frequent clinical observations of adverse renal outcomes particularly AKI, among patients presenting with MI, often with delayed access to care. Participants were provided with clear verbal and written information about the purpose and relevance of the research. Although patients were not directly involved in data analysis or interpretation, their clinical trajectories, delays in presentation, and management challenges were central to shape the study direction. We plan to share the study findings with participants and the broader community through local health education initiatives in the Nepali language, aiming to promote awareness about the risks of delayed presentation and kidney complications after AMI.
