: All study procedures were conducted in accordance with the Declaration of Helsinki and its later amendments, and with all relevant guidelines and regulations governing human subjects research. We received ethical approval from all participating Intuitions of the CRIC consortium, including SDCC and University of New Mexico Institutional Review Board (UNM-IRB). We used the same basic process to consent participants across all sites. The informed consent document was structured such that it enabled potential participants to indicate which aspects of the study they were not willing to engage in (e.g., genetic study). The consent covers all aspects of screening, baseline testing and subsequent follow-up visits. A separate section and signature page was used for consent to collect a blood sample for genetic testing and storage of DNA. The current submission used archived biological materials from representative samples of the CRIC cohort with their respective clinical data. All CRIC participants signed the IRB approved consent and received partial compensation.
