: This case report was prepared as part of a study titled “Investigation of Treatment and Prognosis of Monoclonal Gammopathy of Renal Significance,” approved by the Ethics Committee of Jikei University School of Medicine (approval no: 36–148[12257]). The study was performed in accordance with the 1964 Helsinki declaration and its later amendments or comparable ethical standards. The requirement for informed consent from the patients was waived because of the retrospective nature of the study.
