: In the multiple studies comprising this post hoc pooled analysis, the protocols were approved by central and local ethics committees and the review board at each of the multiple sites (see Supplementary Table  in supplementary information for details). Patients provided written informed consent before participating in the trials. All studies were carried out in accordance with the International Conference on Harmonisation Guidelines for Good Clinical Practice and the World Medical Association’s Declaration of Helsinki and its amendments.
