: The Regional Ethical Review Board in Stockholm (from 2019: The Swedish Ethical Review Authority) approved the study (2017/937 − 31) in line with the Ethics Review Act (legislation number 2003:460). Informed consent was obtained from all participants. All methods were carried out in accordance with the Helsinki declaration. The patients were informed both orally and in written form about their right to withdraw at any time without the need for giving a reason. No illiterate patients were involved in the study.
