Ethics approval and consent to participate
: The Global Health Survey was done during regular rounds of data collection within the KEMRI/CDC’s Health and Demographic Surveillance System (HDSS) area. Only verbal consent was obtained for the additional services related questions. Parental consent for 70 adolescents below 16 years was waived because these adolescents were emancipated minors as a result of them being married and/or orphaned thereby becoming heads of households. The National Commission for Science, Technology and Innovation the commission mandated to regulate research conduct in Kenya has made this provision [CitationRef removed]. The HDSS protocol and consent procedures were approved by both local and international Ethics and scientific review boards. The review for this survey was done by the KEMRI’s Scientific and Ethics Review Unit (SERU protocol #1801] and Center for Disease Control Institutional Review (CDC Protocol #3308) boards. Participants were informed that taking part in the interview was voluntary and could opt out at any time.
