Ethics approval and consent to participate
: Study ethical approval was gained from NHS Research Ethics Committee (NRES) West Midlands-South Birmingham REC (16/WM/0029), the University of the West of England, Bristol Research Ethics Committee (HAS.16.08.001) and the respective study site’s National Health Service (NHS) Research & Development (R&D) departments.
