Ethics approval and consent to participate
: The PIFI trial was approved by the Curtin University Human Research Ethics Committee (HR 82/2014; 14 May 2014) and the Human Research Ethics Committees responsible for the public (SCGG HREC No 2014–111; 18 Sept 2014: WNHS HREC No 2016037EW; 4 May 2016) and private (SJGHC Ref: 777; 8 April 2015) hospital sites. Participation was voluntary and all participants provided signed informed consent. The qualitative component with the FFAB class facilitators was approved by Curtin University Human Research Ethics Committee (HR 82/2014–02; 19 January 2017). All facilitators were emailed a Participant Information Sheet explaining the study, and the study aim was outlined and informed consent obtained verbally prior to commencement of interviews.
